When you request a conversation, we collect the contact details, coverage preferences, target date, and contact-consent choice you provide. This form is for an initial inquiry, not an insurance application. Please do not submit medical or payment information.

How your inquiry is handled

Your request is saved in a private inquiry inbox for Mikias Alem. Authorized website administrators can access records to operate and support the service. Only Mikias will follow up about your coverage inquiry. Your details are not sold or shared with other agents for marketing.

Website hosting and storage providers process the request. Once phone notifications are connected, Pushover processes a brief alert containing your name, state, coverage category, requested date, and a link to the protected inquiry. Full contact details remain in the private inbox.

Your contact preference

The optional checkbox records whether Mikias may text and email you, including marketing messages. You may submit without opting in. Without that consent, your request is for an email reply about the inquiry only. You may reply STOP to texts or unsubscribe from marketing emails at any time.
